An application for support measures or services from the services for persons with disabilities is made in writing. The application must be accompanied by a statement from the health centre doctor and, if necessary, other expert opinions and reports. For more information on housing services reserved for persons with severe disabilities or persons with intellectual disabilities, please contact social workers of disability services.
